Transition into adulthood is a difficult and daunting process to navigate for young people with special educational needs and disabilities (SEND) and their families.

We understand the worries that keep parents awake at night when thinking about the future and searching for the right care and support that will give your loved ones a quality of life. 

What’s Next After Education?

What’s right for one person is by no means right for everyone. It is important to plan ahead and research all the options to make an informed decision on the next step of moving into adult services.

A fulfilling adult life

At Home From Home Care, we do not offer a ‘one size fits all’ approach, as we design completely bespoke environments and support packages around each individual based on their needs to enable them to lead a fulfilled adult life.

We creatively support individuals to achieve their goals and aspirations, building on the life skills and development at school and college into an adult placement, with a focus on;

  • Enjoying the world on their own terms
  • Supporting access and interaction with the wider community
  • Encouraging active support
  • Promoting independence, self-worth and new opportunities
  • Living an ordinary life: something many of us take for granted!

Family Communications

Family involvement does not stop when a loved one moves into our service. We continue to work with our families and ensure communication channels are kept open to enable the sharing of information. We also encourage feedback to understand any area for improvement in a variety of ways;

  • Annual family questionnaire
  • Regular parent calls with our dedicated family liaison
  • Family update films and newsletters throughout the year
  • HFHC Events twice a year for families to come together 
  • Access to a family portal
